Wednesday, April 23rd – a Doozy

It was a very long night last night. Heather noticed that Dallas seemed bothered early in the evening, and it continued past midnight. By then, her heart rate had climbed to a consistent 130–140s, and the nurses were called in. Her temperature was also elevated to 99.1, and she just seemed very uncomfortable — and then she let out a yell (an actual loud yell), far louder than we’ve ever heard her voice in weeks.

By 2 a.m., Dallas was still uncomfortable. She had a BM and then vomited. Zofran (a medication for nausea and vomiting) was given, and an EKG machine was brought in to monitor her heart, which was now sitting steadily in the 150s–160s. I drove back over from the Ronald McDonald House, and we stayed up pretty much until daylight trying to figure out what was going on. Somehow, Dallas eventually settled into sleep — even with her heart rate still consistently around 160.

Through the morning, her other vitals (blood pressure and O₂) remained pretty normal and consistent with her baseline. Blood panels were completed, a viral panel was done, and a chest X-ray was also performed. No viruses were detected. Bloodwork showed a lower red blood cell count and a higher white blood cell count — though the doctors didn’t seem too concerned by the numbers — and the X-ray showed that both the central line and NG tube were still well-positioned. Her nurses also changed out the central line bandages, since vomit had gotten on them last night.

They were able to get a little fluid/discharge from her pre-auricular pit (a little dimple near her right ear that she was born with); it seemed tender to the touch yesterday and a bit swollen and red. They’ve started her on an antibiotic for it, just like her pediatrician did back in January when we noticed similar swelling.

Later in the day, she had her CHG bath and settled in for a nap. We didn’t do her PLEX therapy today, as the team wanted a clearer idea of what was causing her heart rate to stay so elevated — she essentially completed a marathon, heart-rate-wise, for nearly 18 hours. PopPop drove up for some much-needed relief (we haven’t slept in a couple of days), and we were relieved to see her heart rate consistently in the “one-teens” as we were heading out.

The nurses plan to collect a urine sample this evening to run a culture, checking for a potential UTI, as we still don’t have a clear picture of what caused the heart rate spike all night and into the day. Our hope is that her heart rate stays down and that she and PopPop can have a peaceful, restful evening — we all need it. We’re optimistic that she’ll be in a better position for PLEX tomorrow and, if so, could potentially have it again on Friday — getting back on schedule.
